Your first visit at Henderson Smile is about meeting you, not selling you. We block 75 minutes for new patients — long enough for X-rays, a thorough exam with Dr. Park, and a clear conversation about what (if anything) you need.
What to bring
- A photo ID and your insurance card
- A list of any medications and supplements you take
- Any recent dental records or X-rays (we can request them if you don't have copies)
- Questions — bring them written down if it helps

Standard medical and dental history. Takes about 10 minutes. We email it the day before so you can do it at home.

Required for any dental office. Signed once.

Insurance verification
Optional but recommended. We'll call your carrier and show you your coverage breakdown before your appointment if you give us 48 hours.
